Working closely with your ACO, we customize the criteria for patient referrals to align with your population health platforms and care management strategies. We manage the entire process—from patient evaluation and insurance verification to enrollment. Following enrollment, patients receive our In-Home Neuro Care Solutions with a comprehensive Home Neuro Rehab Kit, complete with setup assistance, and meet their personal therapy care team.
After an assessment by one of our licensed therapist, the patient is provided with a 8 to 16-week personalized therapy program, integrating post-acute and chronic care management principles. Progress is monitored weekly through our Remote Therapeutic Monitoring system. Following graduation, patients are offered a maintenance program, supporting value-based care solutions.
Our team collaborates closely with your existing providers, providing continuous updates on patient progress and therapy milestones, aligning with value-based care goals. Our reports integrate with your Neurological Care Management systems, ensuring all updates are reflected in the patient's medical record.
